Every week, we share what we call, "Tip Tuesday." These are bits of best practices to help awesome clinicians even better in documenting. We came about calling them tips because they are not anything new but simple reminders of the simple things that tend to be overlooked. We hope you find our tips refreshing.

The homebound reason, just as it is critically important, must be consistent from the start of care. Varying reasons in documentation may display uncertainty of the assessment of the patient's homebound condition or lack of care coordination. In some instances, the homebound reason may change during the episode. Be sure to report and document either the progress or decline to support the change in the homebound reason. It may call for an update of the plan of care.
